INSTRUCTIONS TO APPLY
Follow instructions below and submit application by Monday, May 5, 2008. Deadline has been extended from April 30, 2008. For more information, call Susanna at 206.280.8537 or e-mail scholarship@jpaulmanjarrez.org .
The completed application must include the following typed in 12-pt font.
1. Provide the following information.
a. Applicant Name
b. Home Address
c. City, State, Zip
d. Phone Number
e. Email Address
2. Type each question in order, then respond to each question below in 150 words or less.
a. How do you embody the ideals of this scholarship?
b. How has your community involvement served youth?
c. What other ways have you served your community?
d. What leadership have you demonstrated through involvement in athletics?
e. What adversities have you overcome through your involvement in athletics?
f. What are your post-high school educational plans?
g. Describe your athletic activities over the past 4 years (school, boys and girls club, recreational leagues, etc.)
h. What benefits have you received from participating in sports?
3. Request two reference evaluations. Provide your references with Reference Evaluation Instructions. Reference evaluations must be submitted directly by the evaluator to the scholarship program. The first evaluation must be from a coach and the second evaluation from another coach or teacher, counselor, pastor, youth leader, etc. Reference evaluations must not be written by applicant's parents or relatives.
4. List awards and honors that you have received.
5. Submit completed application in either method listed below by May 5, 2008.

6. By submitting your application, you hereby give consent to the James Paul Manjarrez Charitable Association to verify you informantion and school transcript.
