INSTRUCTIONS TO SUBMIT REFERENCE EVALUATION FORMS
Reference Evaluation Forms must be submitted directly by the coach, teacher, counselor, pastor, youth leader, etc., to the scholarship program. Evaluations must not be completed by applicant's parents or relatives.
Reference Evaluation focuses on the following characteristic elements and must be submitted by Thursday, May 12, 2011.

Reference Evaluation Form can be obtained via any of the options below.
- E-mail to scholarship@jpaulmanjarrez.org for an electronic version.
- Copy content of the Reference Evaluation Form provided below to a WORD file or to an e-mail message.
- Printed Reference Evaluation Forms are available at Newport High School (NHS) Counseling Center.
Completed Evaluation can be submitted by:
Drop off at NHS Counseling Center. Please sign on the Evaluation Form and provide your contact info (phone number and/or e-mail address) for verification.
