INSTRUCTIONS TO SUBMIT REFERENCE EVALUATION FORMS
 
Reference Evaluation Forms must be submitted directly by the coach, teacher, counselor, pastor, youth leader, etc., to the scholarship program.  Evaluations must not be completed by applicant's parents or relatives.
 
Reference Evaluation focuses on the following characteric elements and must be submitted by May 5, 2008.
  1. Teamwork
  2. Spirit
  3. Dedication
  4. Commitment
  5. Inspiration to others

 

Content of the Reference Evaluation Form is posted below.  Copy it to a WORD file or to an e-mail message and complete.  Printed Reference Evaluation Forms will also be available at Newport High School (NHS) Counseling Center by March 20, 2008. 

 

Completed Evaluation can be submitted by either method listed here.

  1. E-mail to scholarship@jpaulmanjarrez.org with subject titled "Reference Evaluation for <put applicant's name here>"
  2. Drop off at NHS Counseling Center.  Please sign on the Evaluation Form and provide your contact info (phone number and/or e-mail address) for verification.
